Responsibilities
What You’ll Do:
- Collaborate on a multidisciplinary team to coordinate care and services for members
- Conduct comprehensive assessments and develop individualized, member-focused care plans
- Use critical thinking to identify barriers, set goals, and drive measurable health outcomes
- Navigate a range of healthcare settings (hospital, rehab, SNF, home care, etc.)
- Engage members in their care with proactive education and supportive interventions
- Monitor progress, adjust care plans, and document everything thoroughly in EMR systems
- Utilize community and funding resources to support member needs
- Evaluate medical necessity and recommend alternate care solutions when appropriate
✅ What You’ll Bring:
- Active RN license
- 3+ years of bedside/hospital clinical experience
- Previous case management or managed care organization (MCO) experience preferred
- Familiarity with electronic medical records (EMRs) and care coordination systems
- strong critical thinking, time management, and communication skills
- Independent, self-motivated, and comfortable working in a fully remote environment
- Comfortable navigating and learning internal systems (we’ll train you!)
- Bonus: CCM certification or willingness to obtain within 2 years
